As I recall, it was no charge.

How much do you pay, monthly for the service (SMS + Data)?

$10, plus $2/MB for data. It's just added as a phone on our family plan, which includes unlimited texting for all phones1

How much do you pay, per-MB for data?

$2. I used 5 MB on my most recent bill, so $8.

Long-term viability - do you think this is still going to be possible in a year or two?

Yes.
