[Ovmsdev] RFC: CRTD 4.0 -- add support for RTR frames

Mark,

I was suspecting my car could use RTR frames, but missed that in the 
CRTD format, so I've added support.

My proposition for the implementation & new CRTD specification is in 
branch "rfc-crtd4". Please have a look.

I also did some rework / tidying on the documentation.
